Positive Behaviour Support and Dementia

Positive Behaviour Support (PBS) is an evidence-based, non-pharmacological approach increasingly used to address challenging behaviours in people with dementia. PBS focuses on understanding the underlying causes of behavioural and psychological symptoms of dementia (BPSD) and implementing proactive, person-centred strategies to enhance quality of life and reduce distress.

Assessment and Understanding: 

PBS begins with identifying and analysing the triggers and functions of challenging behaviours, such as agitation or aggression, often using tools like A-B-C (Antecedent-Behaviour-Consequence) charts.

Individualised Interventions: 

Strategies are tailored to the individual’s preferences, history, and needs, aiming to create supportive environments and routines that minimise distress and promote well-being.

Collaboration: 

Effective PBS involves collaboration between support staff, clinical leaders, and family members, ensuring consistent and holistic support across care settings.

Training and Capacity Building: 

Studies show that PBS training increases the confidence and skills of caregivers and staff, enabling them to better manage behaviours and improve relationships with people living with dementia. However, ongoing training and system-level support are crucial for successful implementation.

PBS is recommended as a first-line intervention for BPSD, often preferred over medication due to its focus on non-pharmacological, strengths-based, and person-centred care. In Australia, PBS is also mandated under the National Disability Insurance Scheme (NDIS) for individuals with dementia under 65.

Challenges remain, such as the need for adequate resources, specialist skills, and effective systems to translate PBS principles into daily practice, especially given the progressive nature of dementia. Nonetheless, PBS offers a promising framework for improving the quality of life for people with dementia and their caregivers.
